Pest Control Program Failed to Address Flies in Resident Room

Summary

The facility failed to maintain an effective pest control program to prevent or address flies in resident areas. Review of the facility policy showed the pest control program was to provide frequent treatments, staff were to monitor the environment for potential concerns, and pest control problems were to be reported promptly to the Director of Maintenance. However, survey observations found flies in Resident 73's room on multiple occasions, including two flies near the resident's feet and on the windowsill, and later four flies flying around the resident and landing on the resident's right leg, overbed table, and wheelchair. A fly swatter and a dead fly were also observed on the resident's overbed table. Resident 73 was admitted with right-sided hemiplegia and aphasia and, on the comprehensive assessment, was documented to have moderate cognitive impairment and to require assistance from one to two staff members for grooming, dressing, toileting, transfers, and mobility. During interviews, the contracted pest control vendor stated they had not received complaints about flies, Staff I stated they were not aware of flies in resident rooms, and Staff J stated Resident 73's room had flies since July 2025. The Administrator stated they were surprised to learn the resident had been dealing with flies for that length of time, and the Maintenance Director stated they were aware of flies in the room in about September and had likely given the resident a fly swatter, but had not followed up to determine whether the flies were gone.
