Summary

The facility failed to offer and/or provide an influenza immunization for 1 of 5 residents reviewed for immunizations and infection control. The resident was admitted with diagnoses including anxiety, depression, and necrotizing fasciitis, and the comprehensive assessment showed the resident was cognitively intact and required assistance from one to two staff members for activities of daily living, including setup assistance for eating and oral hygiene. The facility policy stated residents were to be offered influenza immunization between October 1 and March 31 each year unless medically contraindicated or already immunized during that period, and the medical record was to include documentation of education, receipt, or refusal. Review of the resident’s medical record showed no documentation of consent or refusal for the annual influenza immunization, no record that the immunization had been administered, and no documentation that education had been provided. The Infection Preventionist stated the resident was not offered the immunization upon admission because the resident was admitted in July and was unsure why the resident was not offered it in September when all residents were offered it. The DON stated the immunization was offered to all residents on admission and during respiratory season, that consents were obtained from all LTC residents prior to the start of influenza season, and that the resident was missed because they were in and out of the hospital when consents were obtained.
