Summary

The facility failed to follow professional standards of practice when it added a new mental health diagnosis for Resident 31 without a documented mental health evaluation. Resident 31 was admitted with diagnoses of dementia and affective disorder and was receiving Risperidone 1 mg twice daily for dementia without behavioral disturbance. The comprehensive assessment showed severe cognitive impairment and dependence on staff for grooming, dressing, incontinent care, bathing, and mobility. Provider progress notes from the admission period showed the resident was evaluated to establish care and review medications, but there were no changes to the medication regimen or diagnosis list. A pharmacist consultation report in October 2025 recommended clarifying the indication for Risperidone because the listed diagnosis of dementia without behavioral disturbance was not appropriate. Staff L, ARNP, wrote bipolar on the report and initialed it, and a physician order was then entered for Risperidone 1 mg twice daily for bipolar disorder. However, the resident’s diagnosis list was not updated, and the medical record later showed no monitoring for behaviors related to bipolar disorder. During interview, Staff L stated they had not evaluated Resident 31 specifically for mental health before adding bipolar disorder, and Staff B, DON, stated the standard of practice was to obtain a mental health evaluation before adding a new mental health diagnosis, which did not occur for Resident 31.
