For Resident R31, the record showed he was admitted with diagnoses including high blood pressure, diabetes, and pain. A nurse practitioner note documented that he became unresponsive after receiving an incorrect opioid medication by error, including oxycontin that was not ordered for him, along with his scheduled Percocet doses. He required 1 mg of Narcan after staff found him lying face down on his bed and difficult to awaken.
