Inconsistent meal portions and failure to follow posted menus
Summary
Dietary services failed to ensure that residents received the planned portion sizes and the pre-approved menu items that were posted and prepared for service. Multiple residents reported that meal portions were small and inconsistent, that they did not receive enough food, and that requests for substitutes or seconds were denied because staff said items had run out. One resident who was supposed to receive double portions stated he often received less food than others at the table, and another resident reported that meals did not match the tray ticket and that substitute choices were unavailable. Observations confirmed inconsistent serving sizes and tray contents. One resident’s lunch tray contained only one piece of Salisbury steak, one scoop of macaroni and cheese, one scoop of vegetables, and a very small piece of cake, while another resident’s tray ticket indicated 12 ounces of chicken and dumplings but the tray contained only one dumpling with no chicken or gravy and a small dinner roll. At the same time, another resident received a larger portion of the same entrée despite a tray ticket for 6 ounces. The dietician confirmed that the serving sizes were not consistent and that the dinner rolls were very small because they were not permitted time to rise. The posted menu and actual meal service also did not match. The lunch menu posted for one meal listed stuffed bell pepper, mashed potatoes, Italian blend vegetables, dinner roll, and angel cake with fruit topping, but the kitchen served stuffed pepper with garlic bread, and the pureed meal available was pureed beef tips and pureed creamed corn. The alternative meal request form required lunch changes to be submitted by 9:30 a.m., yet the menu changes were not posted for residents to see and residents were not informed verbally. The dietician and regional dietary manager stated that the menu should have been posted before the deadline, that the pureed lunch item was roast beef according to the extension menu, and that residents on pureed diets were not provided a menu or information about receiving something different from the posted menu. The regional dietary manager also stated that prepared meals were being reheated in an oven because the hot box had been broken, and that serving sizes on some items varied and were not consistent.
