Food Service Sanitation and Kitchen Cleanliness Deficiencies

Summary

Food service operations in the Main Kitchen were not maintained in accordance with professional standards, as the facility failed to properly label and date food, clean and sanitize food service items and dishes, and keep the kitchen clean. The Dietary Services policy, Sanitation dated 9/12/25, stated that food and nutrition services staff would maintain a clean and sanitary environment in food service areas through compliance with a written, comprehensive cleaning schedule. During an observation of the Main Kitchen on 1/6/26 at 9:05 a.m., surveyors found that the dishwasher was not reaching temperature to effectively sanitize, the plate warmer was not operational, and there was a hole in the ceiling related to a pipe burst that was covered with plastic after the pipe was fixed. Styrofoam was being used to serve residents, and the Dietary Manager confirmed it was being used because the dishwasher and plate warmer were not working. On 1/7/26 at 1:48 p.m., the kitchen still had a hole in the ceiling with two fans placed in it and extension cords hanging down, a dehumidifier was in the kitchen to remove excess moisture, and the drainage hose from the dehumidifier was wrapped around the faucet and routed to the three-compartment sink drain. The Dietary Manager confirmed the dehumidifier and fans had been placed by the restoration company after free-standing water was found behind the wall. On 1/15/26, the Nursing Home Administrator confirmed the facility failed to properly label and date food, clean and sanitize food service items and dishes, and maintain cleanliness in the Main Kitchen and one of two nursing unit nutrition rooms.
