Summary

Surveyors determined that the facility failed to provide a safe, sanitary, and comfortable environment in the main kitchen and on one nursing unit. During an observation period from 12:00 p.m. to 1:00 p.m. on January 29, 2026, the mop sink in the main kitchen janitors' closet was found to be clogged with an accumulation of black sludge around the base of the sink.
