Summary

The facility failed to ensure that necessary behavioral health care and services were provided to one resident with a history of suicidal behavior. The resident was admitted with diagnoses including a personal history of suicidal behavior, and the hospital history and physical before admission documented that the resident had been found by a family member with self-inflicted lacerations to the left wrist and scissors present in the bed. Although the resident denied suicidal intent at that time, the documented history showed a need for ongoing assessment and behavioral health monitoring. The resident's care plan, initiated after admission, did not include individualized interventions or goals to address the history of suicidal attempts or suicidal ideation, and it did not address the documented behaviors, risks, or need for behavioral health monitoring. The record also showed that the resident's son died by suicide, and although the resident was seen by an outside psychiatric provider, the care plan was not updated through the survey exit date to reflect this significant psychosocial event or the increased risk for mental health decline following the loss of a child. During interview, the DON was unable to provide evidence that the care plan had been updated to address the resident's history of suicidal behavior, recent family loss, or the need for behavioral health interventions.
