Summary

The facility failed to ensure that Resident 5’s drug regimen was free from unnecessary drugs when digoxin was administered without documented pulse monitoring before each dose. Resident 5 was admitted with diagnoses including atrial fibrillation, and a 12/19/25 physician order directed digoxin 250 mcg once daily. The manufacturer recommendations stated the heart rate should be monitored for one full minute before administration and the health care provider notified if the heart rate was less than 60 beats per minute or if there were significant changes in pulse rate, rhythm, or quality. The clinical record did not show that staff checked Resident 5’s heart rate each time before giving digoxin. Staff 7 stated she held the medication if the pulse was below 60 but did not always check the pulse before administration, relying instead on recent CNA-recorded vital signs in the electronic health record. Staff 9 stated Resident 5 did not have digoxin parameters noted in the electronic health record and that she used CNA vital signs recorded within the last hour to decide whether to administer the medication. Staff 2 acknowledged the order had no parameters noted in the clinical record and stated staff were to check the resident’s pulse before administering digoxin and hold it if the pulse was too low.
