The facility failed to ensure complete transfer or discharge information for one resident who was hospitalized after an outpatient provider requested emergency room evaluation. Resident #12 was admitted with diagnoses including acute respiratory failure with hypoxia, hemiplegia and hemiparesis following cerebral infarction affecting the right dominant side, hyperkalemia, type 2 diabetes mellitus, muscle weakness, and muscle wasting.
