The second resident, admitted with COPD, chronic respiratory failure with hypoxia, DM, major depressive disorder, adult failure to thrive, tachycardia, GERD, and essential hypertension, also cognitively intact, reported that she observed the CNA enter the room, open the closet door, and proceed to urinate on a box in the closet with the door left open so she could see his side profile. She stated that she began screaming loudly, saw urine on his pants, and felt very upset, scared, unsafe, and confused about why he would do this. Facility staff, including the HR manager, confirmed that the CNA urinated in the residents' closet, and documentation in the employee record identified the conduct as a violation of the facility’s code of conduct and dignity policies.
