Insulin Pen Not Primed Before Administration

Summary

The facility failed to ensure residents were free from significant medication errors when staff attempted to administer insulin using an incorrect technique for one resident. The resident had diagnoses including diabetes, morbid obesity, COPD, metabolic encephalopathy, and kidney failure. The resident’s care plan identified a problem of being at risk for elevated or low blood sugar levels related to diabetes, with interventions for diabetes medication as ordered and monitoring/documenting side effects. The physician order sheet showed an order for Aspart Flex pen insulin, 3 units three times a day. During observation, an LPN checked the resident’s blood sugar, which was 149, verified the order for 10 units of Aspart insulin, and went to the medication cart to obtain the insulin pen. The LPN verified the resident’s name on the pen and dialed it to 10 units without priming the Flex pen with 2 units as required by facility policy. The LPN nearly administered the insulin before being stopped when the pen showed 50 units of air and was leaking. During interview, the LPN stated, “This is how we do things all the time.” The ADON stated the insulin should have been primed at 2 units per policy and said the resident may not have received the full dose or any insulin because of the air inside the Flex pen. The ADON also stated the pen needed to be replaced and that the amount of air indicated the pen was not calibrated correctly.
