Summary

Safe, appropriate pain management was not provided for a resident with chronic osteomyelitis with drainage sinus involving the right tibia and fibula, chronic pain most severe in the right knee, muscle weakness, and difficulty walking. The facility’s pain management policy required staff to recognize pain, assess it, and manage it according to the resident’s condition and preferences, including observing for nonverbal indicators and using a pain scale. The resident’s care plan included giving medications as ordered for pain and monitoring for excessive pain, yet the record showed repeated pain assessments and treatment documentation that did not reflect effective follow-through on the resident’s prescribed narcotic pain medication. The physician ordered Percocet 10/325 mg for right knee pain, first as a three-times-daily order for two weeks and then as 1 tablet every 8 hours as needed. However, the pharmacy representative stated no order had been received from the facility, and the MAR showed no Percocet was administered. During interviews, the resident reported that staff were not properly caring for the right knee wound, that the dressing had not been changed as expected, and that staff were only giving Tylenol or ibuprofen, which did not help the pain. The resident also stated he/she stopped asking for pain medication because staff were not providing it, and later reported pain levels as high as 9/10 and 10/10. Observation of the wound showed the dressing was not dated, initialed, or timed, was coming off, and the wound had discoloration, purulent and serosanguinous drainage, erythema, and yellow drainage with slough. Interviews with nursing staff and the RN supervisor showed confusion and breakdowns in the process for handling controlled substance orders, including faxing orders to the pharmacy, verifying receipt, and following up on new medications. One nurse stated the Percocet order was found and refaxed later, while another said the pharmacy never received the fax because another resident’s prescription had been faxed twice. The RN supervisor stated that 12 days without addressing the resident’s pain was excessive and that the chart should have triggered a pain assessment, but the resident’s record showed no Percocet administration during the period reviewed.
