Incomplete smoking assessments and lack of supervision for supervised smokers

Summary

The facility failed to complete smoking assessments for two residents and failed to evaluate their smoking behavior risks. Resident #7 had intact cognition and diagnoses of schizoaffective disorder and delusional disorder. The resident’s care plan identified the resident as a supervised smoker, with interventions stating the resident required supervision while smoking, but the Smoking and Safety Evaluation dated 4/25/25 was incomplete with only one question answered and the evaluation dated 6/12/25 was blank. Resident #108 also had intact cognition and diagnoses including schizoaffective disorder, unspecified disorder of psychological development, anxiety, personality disorder, and drug induced subacute dyskinesia. The resident’s care plan likewise identified the resident as a supervised smoker, but the Smoking and Safety Evaluation dated 3/12/25 was incomplete and the evaluation dated 6/12/25 was blank. During observation, both residents were resting in bed in a room where the floor had dirt around the perimeter, the windowsill had ash-like substance over more than half of the ledge, the window was open about six inches, and there were 13 cigarette butts and cigarette ash in the window tracking. Ash-like substance was also observed on the floor in front of the window. Later, CNA H and ADON A stood in the doorway, stated the room smelled like cigarette smoke, and asked whether anyone was smoking; Resident #108 denied smoking, and both staff members left without further investigation. The window remained open with cigarette ash in the tracking, and the cigarette butts were later no longer present. The Administrator stated staff were expected to follow the smoking policy and report concerns immediately to the charge nurse, and that all residents who smoke were expected to have a smoking assessment completed annually and quarterly.
