Delayed Notification of UTI Lab Results

Summary

The facility failed to ensure prompt physician notification of laboratory results for two residents with suspected urinary tract infections. The deficiency involved urinalysis and culture/sensitivity results that were completed by the laboratory but were not communicated to the ordering practitioner in a timely manner. The facility policy titled Antibiotic Stewardship Program stated that when a UTI is suspected, the resident’s clinical assessment should be documented and communicated to the physician, and that the antibiotic stewardship team should ensure accurate completion of related documentation. For one resident, staff documented red-tinged urine and burning, collected a urine specimen, and the final culture report later showed greater than 100,000/ml Klebsiella pneumoniae and greater than 100,000/ml Escherichia coli, both resistant to ciprofloxacin. No further notes were completed regarding urinary status, pending results, or physician notification until five days after the final report, when the DON emailed the results to the physician. The resident later reported urinary burning and passing blood over the prior couple of weeks, and an order for ciprofloxacin was then entered. For the second resident, staff documented red-tinged urine and burning, collected a urine specimen, and the final urinalysis/culture showed greater than 100,000/ml Klebsiella pneumoniae and greater than 10,000 CFU/ml Enterococcus faecalis, with resistance to Macrobid and susceptibility to multiple antibiotics. The DON did not send the results to the physician until four days after the final report. The resident’s record later showed an order for Macrobid for UTI, and staff interviews indicated that lab results were expected to be checked and sent to the physician promptly, with pending labs monitored through the lab website or shift report.
