Infection Control Lapses During Wound Care and Incontinence Care
Summary
The facility failed to provide and implement an infection prevention and control program when staff did not consistently change gloves between dirty and clean tasks, did not complete appropriate hand hygiene, and did not properly handle soiled linens and trash for four residents. The cited policy required hand hygiene before donning gloves and immediately after removing them, and required used linen to be handled as potentially contaminated, collected at the bedside, and placed in a linen bag or designated lined receptacle rather than being left in the room or placed on other surfaces. For one resident with MRSA, a surgical wound, and enhanced barrier precautions for a chronic wound, the ADON/wound care nurse performed wound care to the left hip surgical site. After removing the old dressing and taking off gloves, she wrapped the dressing in the soiled gloves and placed them in the resident’s trash can, then put on a new pair of gloves without performing hand hygiene. She completed the wound care and only performed hand hygiene after removing the final gloves. During interview, she stated she should have performed hand hygiene between glove changes and did not because sanitizer was not readily available, although she could have used soap and water. For another resident who was frequently incontinent and required assistance with toileting hygiene, a CNA checked for incontinence, found urine, and used the same soiled gloves while cleaning the resident’s peri-area, rolling the resident, removing the brief, placing a clean brief, and handling soiled washcloths and the urine-soaked brief. The brief was placed on the floor, and the soiled items were later bagged and taken to the dirty utility room. The CNA did not clean the floor where the soiled items had been placed and stated he/she forgot to wash hands or use sanitizer after removing gloves. For two additional residents, CNAs used soiled gloves while moving from dirty to clean tasks, placed used washcloths, dirty briefs, and towels on a bedside table, floor, or wheelchair seat instead of immediately bagging them, and then touched clean items, bed controls, door handles, linens, and other surfaces without changing gloves or performing hand hygiene. Staff interviews confirmed that dirty linens should not be placed on the floor, wheelchair, or bedside table, and that hand hygiene and glove changes were expected between dirty and clean care.
