Summary

The facility failed to store food brought in from outside sources in a safe manner for Resident #121. The facility policy titled Food Brought by Family/Visitors, revised March 2022, required food brought by family or visitors to be handled using safe food practices, including proper cooling and reheating, labeling, and storage of perishable foods in a refrigerator in re-sealable containers with tightly fitting lids. The policy also stated that potentially hazardous foods left out without heat or refrigeration for longer than 2 hours are to be discarded. Resident #121 was admitted in October 2025 with diagnoses including cancer and malnutrition, and the MDS dated 10/29/25 showed intact cognition. On 11/18/25, the resident was observed in bed with old food in a container that appeared to be from family or a visitor, along with two other food containers on the nightstand. On 11/19/25, the resident was again observed with two containers of food on the nightstand and an open container of ramen soup with a chicken thigh inside, and stated the food had been there since the previous night and that the resident orders food out. The Unit Manager said the resident sometimes gets food from outside and keeps it in the room because the resident does not want to place it in the unit refrigerator. The Food Service Director stated that outside food leftovers should be labeled, dated, and placed in the unit refrigerator, and later stated the family had been bringing in food and the facility planned to reach out to the family to get a refrigerator for the resident's room.
