Unvaccinated Resident Dog Allowed in Dining Room

Elkton Nursing And Rehabilitation CenterElkton, Maryland Survey Completed on 10-09-2025

Summary

The facility failed to ensure reasonable clinical and safety restrictions under its policies and procedures by allowing an unvaccinated animal into the controlled environment. Surveyors observed a dog in the 1st floor Dining Room brought in by family, and the dog was later identified as belonging to resident #115 and visiting weekly. During interview, the Director of Recreational Therapy provided the facility policy stating that all animals, including service animals used for therapy and resident animals, are to have vaccination records on file. However, during follow-up interview, staff stated there were no vaccination records on file for resident #115’s dog and that the facility would need to obtain them from the family.

Short Summary

A cognitively intact resident with multiple chronic conditions was denied her preferred visitor after an incident during a bible study activity. The Activities Mgr and ED spoke with the resident's son, who was not her POA, and the son approved telling the friend she could no longer visit; staff then informed the friend without telling the resident. The resident later said she still wanted the friend to visit, and the DON confirmed the son was only the advocate and emergency contact.

D
F0563 F563: Honor the resident's right to receive visitors of his or her choosing, at the time of his or her choosing.
Short Summary

A resident’s long-time friend, a former employee previously terminated over an abuse allegation, was barred from entering the facility when she attempted to visit, and was told law enforcement would be called if she returned. Another individual confirmed awareness of the restriction, expressed no concern about the friend abusing the resident, and stated that the facility did not offer supervised or common-area visits. A staff member reported that any former employee terminated for an abuse allegation was categorically prohibited from returning to the building, without considering the resident’s relationship with the visitor, despite a visitation policy stating residents have the right to receive visitors of their choice and allowing only limited or supervised access when abuse is suspected or found.
