During an interview on 1/8/26 at 9:45 a.m., the Licensed Social Worker reported that she had reviewed the entire record and found several bed-hold notices, but none corresponding to the date of the hospital transfer in question, confirming that the notice was not provided for that hospitalization. This deficiency is based solely on the absence of documentation of a bed-hold notice for the specific transfer date and the Licensed Social Worker’s confirmation during the surveyor interview that no such notice was given for that event.
