The facility failed to ensure a resident was free of chemical restraints for 1 of 5 residents reviewed for unnecessary medications. The resident was admitted with diagnoses including hemiplegia and hemiparesis following cerebral infarction affecting the left non-dominant side, cerebral infarction due to embolism of the right middle cerebral artery, bipolar disorder current episode depressed mild or moderate severity unspecified, generalized anxiety disorder, and depression. Current physician orders included aripiprazole 10 mg daily for bipolar disorder, fluoxetine 20 mg daily for depression, and monitoring for behaviors every shift, antidepressant side effects every shift, and antipsychotic side effects every shift. The resident’s MDS showed a BIMS score of 13 with no cognitive impairment and identified use of antipsychotic and antidepressant medications. The resident’s care plan addressed behavior problems related to bipolar disorder and depression and included interventions to administer medications as ordered and monitor/document side effects, effectiveness, and behaviors such as agitation, restlessness, aggression, delusions, hallucinations, and refusal of care. Review of the November 2025 TAR showed the facility failed to document antidepressant side effect monitoring 11 times, antipsychotic side effect monitoring 11 times, and behavior monitoring 10 times. Review of the December 2025 TAR showed additional omissions of antidepressant side effect monitoring, antipsychotic side effect monitoring, and behavior monitoring 5 times. RN interviews confirmed the missing documentation while the resident was receiving aripiprazole and fluoxetine.

The resident’s RP stated the resident’s sign of restlessness was isolating himself and reported that the resident texted her about having anxiety from 10:30 AM until 7 PM on 6/30/2026. The DON reviewed the MAR and stated it was not accurate because staff had different ideas of what restlessness was, and stated the resident should have the correct manifestation of anxiety reflected in the physician order and MAR. The facility policy stated psychotropic medications are to be used only when a practitioner determines the medication is appropriate to treat the resident’s specific, diagnosed, and documented condition.
