From admission on 04/11/2025 through discharge on 11/22/2025, the medical record contained documentation of only one face-to-face physician visit, dated 05/06/2025, with no additional face-to-face physician visits recorded for the remainder of the stay. During an interview on 01/06/2026 at 3:18 p.m., the DON confirmed that the resident had only one face-to-face physician visit during the entire period from admission to discharge and acknowledged that a face-to-face physician visit should have occurred every 60 days after the initial visit.
