Kitchen Sanitation and Clean Dish Storage Deficiency

Summary

Improper storage of clean dishes and failure to maintain a clean kitchen were identified during observations and staff interviews in the main kitchen serving a census of 37 residents. On 1/20/2026 at 9:10 AM, surveyors observed dried food and liquid splatter on the refrigerator, thick dust on top of the convection oven with dried food splatter, sugar and flour storage bins with dried food particles inside and outside, dusty open shelving where dishes were stored with dried dark liquid noted, pans stored on open shelves not inverted, thick grease and dust on ceiling vents, a fan in the clean dish area blowing on dishes with thick dust, dried food particles on the plastic covering for the metal cart holding cooking sheets, dried food particles on the large mixer, and dried food particles on the countertop food weigh machine. Floors throughout the kitchen were also observed to be dirty. Staff E stated the bins on the outside were cleaned weekly and could not remember the last time they were cleaned on the inside, and noted the containers had open bags that were not dated. On 1/21/2026 at 10:10 AM, the same areas of concern remained unchanged. The Dietary Manager later showed the cleaning list for daily, weekly, and monthly tasks and reported she did occasional audits for cleaning. When shown the observed areas, she acknowledged they needed to be cleaned and was unsure whether staff were properly cleaning them, stating she would have to investigate why the food particles and debris remained from the prior day. Review of the facility cleaning checklists with a revised date of 12/8/25 documented the areas of concern were to be cleaned daily.
