Summary

The facility failed to ensure foods were disposed of after the use-by date, cold foods were maintained at a safe temperature on the salad bar, beard restraints were effectively worn, and employee drinks were not kept in food prep areas during kitchen observations. During an initial kitchen tour, a container of French toast mix was labeled with a use-by date of 8/19/25, and the Food Services Director stated that date was the last date the food could be used and disposed of the mix. During a lunch observation, cottage cheese and hard boiled eggs were placed on the salad bar and served to three residents; at 12:06 p.m., the Food Services Director stated the cottage cheese was 53 degrees F and the hard boiled eggs were 55 degrees F, and removed the food from the salad bar. During a later kitchen observation, an employee placing food onto a plate was wearing a beard cover pulled down below his mouth, exposing his mustache. In another kitchen observation, an unopened energy drink was observed in the food preparation area while condiments were being portioned, and the Food Services Director stated employee drinks should not have been in food prep areas and discarded the drink. The Food Services Director later stated all facial hair was required to be contained in the beard cover. The facility policy indicated cold food should be held at 41 degrees F or below, leftovers must be dated and labeled, facial hair restraints are required in food production areas, and employees may drink only in designated areas or from closed containers handled to prevent contamination of exposed food.
