Based on interview and record review, the facility failed to ensure the Activity Director was certified for 1 of 1 Activity Director reviewed. During interviews, the Administrator stated the Activities Director had recently resigned and the Activities Assistant was currently overseeing activities. The Activities Assistant stated she was not certified and that the facility currently lacked an Activities Director. The Administrator later stated the previous Activities Director’s last day worked was 12/12/25 and that the facility had not utilized a consultant after she resigned. Record review on 1/8/26 showed employee records lacked a current Activity Director. The Administrator also provided a current, undated Nursing Home Activities: Resident Rights and the Role of Activities Staff policy stating the activities program must be directed by a qualified professional who meets specified qualifications.
