Failure to Use Required PPE During Wound Care for Resident on EBP

Summary

The facility failed to maintain infection prevention and control practices to prevent the transmission and/or development of infection for one of three sampled residents reviewed for Enhanced Barrier Precautions (EBP), R15. The facility policy titled Enhanced Barrier Precautions, dated 3/1/2025, stated that EBP uses targeted gown and glove use during high-contact resident care activities, including wound care for any skin opening requiring a dressing. R15 was admitted on 7/24/2023 and had diagnoses including a pressure ulcer of the sacral region, tracheostomy status, and gastrostomy status. The quarterly MDS dated 10/27/2025 showed severe impairment in cognitive skills for daily decision-making, short-term or long-term memory problems, total assistance with ADLs, one unhealed Stage 3 pressure ulcer/injury, and a tracheostomy. The care plan indicated R15 was on EBP related to a PEG tube, tracheostomy, and wounds, with staff directed to utilize PPE per facility protocol. During an observation on 12/31/2025 at 2:05 pm, a sign for EBP was posted on R15's room door, and wound care was observed being provided to R15's left upper buttocks by LPN7 and LPN8. Neither LPN donned a gown before or during the wound care. In immediate interviews after the care, both LPN7 and LPN8 stated they did not don gowns, that they should have worn gowns, and that it was an oversight. Later interviews showed LPN8 stated gloves, gowns, masks, and goggles, if required, should be worn for a resident on EBP, and LPN7 stated gowns and gloves were required for residents on EBP. The DON stated hand hygiene, gloves, and gowns were required when providing care to a resident on EBP, and the Administrator stated staff should have followed the proper protocol and used the proper PPE if required for resident care.
