Summary

The facility failed to include discharge goals in the comprehensive care plan for one of four sampled residents, R104. The facility policy titled Comprehensive Care Plans, revised 3/1/2025, stated that the comprehensive care plan must include the resident's goals for admission, desired outcomes, preferences for future discharge, and discharge plans as appropriate. A review of R104's admission record showed the resident was admitted on 5/26/2023 with diagnoses including chronic obstructive pulmonary disease, problems related to unspecified psychosocial circumstances, and functional quadriplegia. The quarterly MDS assessment with an ARD of 11/20/2025 showed a BIMS score of 15, indicating intact cognition, and that the resident was dependent on staff for toileting, showering/bathing, dressing, and personal hygiene. The Nurse Practitioner stated the resident had been trying to get out of the facility but had nowhere to go, and the DON and Administrator stated their expectations were that social services would work with the resident and that care plans be accurate.
