Summary

The governing body failed to assume responsibility for the protection of all 187 residents by not acting in a timely manner regarding a failing fire suppression system. During the annual survey on 1/12/2026, the Life Safety surveyor observed that the automatic fire sprinkler system and fire pump were red tagged and had been since 5/5/2025, showing that the fire suppression system would not function as expected in the event of a fire. The report also states there were two additional red tags on each system, dated August 2025 and November 2025. Record review and email correspondence showed the CEO was notified of the fire pump and sprinkler issues on or about 5/9/2025.
