LVN 5 documented administering insulin to six residents at the same time she documented obtaining their blood sugar levels, but she later verified that the insulin was actually given at different times after the residents' meal cart arrived. LVN 5 also acknowledged that she wrote down the wrong room number for one resident's blood sugar result and that the glucometer used did not record the exact time or date of the readings.
