Dietary Staff Failed to Follow IDDSI, Cooling, and Sanitizer Testing Procedures

Summary

The facility failed to ensure dietary staff could safely and effectively carry out food and nutrition services when Cook 1 did not know how to fully test minced and moist texture foods using the spoon tilt test. During a concurrent interview and demonstration, Cook 1 could not show how the spoon tilt test was done and stated she did not know what it was. The Registered Dietician stated the cook should have known how to prepare and check thickened liquids using the spoon tilt test for IDDSI diets, and the facility’s Minced and Moist IDDSI document identified the spoon tilt test as a critical test. This failure had the potential for risk of choking for six residents who had dysphagia and received minced and moist diets from the kitchen during the lunch meal. The facility also failed to ensure dietary staff followed required procedures for cooling ambient food and testing sanitizer concentration. Cook 2 stated that cooling down tuna salad meant cooling room-temperature food to 40 F in two hours, while the RD stated the correct process was cooling from 140 F to 70 F in two hours and then from 70 F to 40 F in the next four hours, as outlined in the facility’s cooling procedures. In addition, a Dietary Aide tested the red bucket Quat sanitizer but dipped the test strip for 6.9 seconds instead of the manufacturer’s required 10 seconds. The Dietary Services Supervisor stated the test strip should have been dipped for 10 seconds and that failing to do so could result in the wrong sanitizer concentration and ineffective sanitization of food contact surfaces. These failures had the potential to cause foodborne illnesses for 111 residents who received food from the kitchen.
