The IP provided LVN 6's competency checklist showing only the PureWick demo cleaning was completed, and stated she did not assess the staff member's competency on the day he was signed off. The IP further verified that the observation of LVN 6 applying the PureWick was done on a different day, but there was no record of when that observation occurred. The Administrator, DON, and ADON were informed of the findings and acknowledged them.
