PPE and N95 Use Not Followed During COVID-19 Precautions

Linden Post AcuteToppenish, Washington Survey Completed on 11-21-2025

Summary

The facility failed to ensure staff followed infection control standards of practice for residents on transmission-based precautions during a COVID-19 outbreak. The deficiency involved staff use of PPE, including N95 respirators, gowns, gloves, and eye protection, when entering COVID-19 positive resident rooms and while performing COVID-19 testing. The report states that multiple staff and residents were COVID-19 positive, and the facility had implemented facility-wide N95 respirators for exposure control. During observation, Staff I wore an N95 respirator with the head straps cut and tied behind the ears, then entered a COVID-19 resident room and placed a second N95 respirator from the resident’s closet door, along with a gown and face shield that had been hanging on a used biohazard container. Staff I stated they preferred to wear two N95 respirators and kept these items in the resident COVID-19 rooms. Staff P was observed wearing a purple mask that was not an N95 respirator while working in resident hallways, and Staff Q was observed wearing a BYD DE2322 N95 respirator while caring for residents. Staff J entered a COVID-19 resident room wearing an N95 respirator, but had a beard protruding about two inches from the mask and stated they would never shave their beard and that the N95 did not form a seal as it should. Staff F was observed performing COVID-19 testing while wearing an N95 respirator over a full beard that crossed under the respirator seal, then donned a gown and gloves. Staff F entered and exited four resident rooms while wearing the same N95, gown, and gloves, but did not wear eye protection during the testing. Staff F stated they had not had a full beard when originally fit-tested and did not currently have a tight seal with the beard. Staff K was observed using a face shield from a medication cart, entering a COVID-19 resident room, then removing gown and gloves, carrying the face shield out, wiping it with a sanitizer cloth, and rehanging it on the cart; Staff K repeated this reuse of the face shield and stated they had no reason why they reused it. The infection control nurse, DON, and administrator stated staff entering COVID-19 positive rooms were required to wear fit-tested N95 respirators, gowns, gloves, and eye protection, and that disposable PPE was not to be reused.

D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident being evaluated for C. difficile was not consistently managed under the correct contact-enteric precautions. Staff gave care with incomplete understanding of the precautions, the room signage did not clearly identify the needed disinfectant or contact time, bleach wipes were not always available, and staff were observed missing hand hygiene and handling items under PPE during care.

Failure to Use EBP PPE During Wound Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Failure to Use EBP PPE During Wound Care: A resident with a wound and severely impaired cognition was receiving ordered wound care when the DON and ADON entered the room, washed hands, and donned gloves but did not wear gowns before starting care. EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

Failure to Follow Enhanced Barrier Precautions and Hand Hygiene During Resident Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.

Failure to Use Required PPE for Resident on Contact Precautions
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A resident on Contact Precautions for active C. difficile infection was not consistently protected by required PPE. Although signage outside the room directed all entrants to wear a gown and gloves, an RN entered the room with medications without either item and touched the bedside table and door surface. The resident stated staff did not always wear gowns, and the DON confirmed staff were expected to wear a gown and gloves when entering the room while precautions were in place.

Infection Control Lapses During Incontinence and Ostomy Care
E
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

A facility failed to maintain infection control during care for two residents. During incontinence care, a CNA removed gloves and applied clean gloves without hand hygiene before placing a clean brief on a resident who was dependent for toileting hygiene and always incontinent. During ostomy care, an LVN handled a resident’s colostomy, wiped stool from the stoma, and continued care without removing soiled gloves, sanitizing hands, or putting on clean gloves. The DON stated staff were expected to perform hand hygiene between glove changes.

Infection Control Failures During Incontinent Care
D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Infection Control Failures During Incontinent Care: Two residents were observed receiving incontinent care with multiple infection control lapses. A CNA did not perform hand hygiene between glove changes or after glove removal while caring for one resident, and another CNA did not wash hands before care, changed nothing between dirty and clean tasks, touched clean items with dirty gloves, placed soiled linens on the floor, and handled dirty linens with bare hands after glove removal. The DON stated hand hygiene, glove changes, and proper handling of soiled linens were required, and facility policies reflected those practices.
