Summary

Facility staff failed to provide complete respiratory services for one resident by not clarifying the oxygen flow rate for as-needed oxygen orders. The resident was assessed as cognitively intact on the most recent MDS and was not assessed as using oxygen at that time. During an interview, the resident was observed with an oxygen concentrator at the bedside and a nasal cannula stored in a closed plastic bag, and stated that she wore oxygen at night sometimes when she felt she needed it. The physician orders included oxygen via nasal cannula or mask at bedtime for comfort and oxygen via nasal cannula or mask as needed for shortness of breath/comfort, but neither order documented a rate of administration. The progress notes stated that a standing oxygen order was initiated per the daughter’s request for comfort at night. The eTAR showed oxygen use at bedtime on 26 of 36 days reviewed, with a flow rate of 1 lpm documented on only 2 dates and no flow rate documented for the remaining dates. The DON stated that oxygen for comfort was used with pulse oximetry monitoring and set to the resident’s comfort, and that the order should have included parameters for nursing staff to follow. The facility standing orders and oxygen administration policy described oxygen via nasal cannula or mask for comfort/SOB at rates per facility protocol, with nasal cannula used to deliver 1-6 lpm and simple mask 5-15 lpm.
