Summary

Facility staff failed to maintain resident room [ROOM NUMBER] in a homelike condition when a section of the wall next to the resident's bed remained damaged and unrepaired. On multiple observations on 10/08/2025 and 10/09/2025, the left side of the bed was against the wall and the covering of the sheetrock was torn away in an area measured approximately 12 inches high by 16 inches wide, later described by maintenance as approximately 16 inches high by 20 inches long. The damaged wall remained visible during each observation. During interview, the maintenance supervisor stated staff could report repairs verbally, by email, phone call, text message, or by completing a Maintenance Request Sheet at the nurse's station, but he checked the sheet sporadically. He also stated he conducted rounds three to four times a week and did spot checks in some resident rooms. When observed with the surveyor, he stated there was a plan to fix the wall but had nothing in writing describing the plan or timeframe. He said the intended repair was to use floor planking and chair rail to create the appearance of wainscoting, and after reviewing the receipt for supplies dated 09/17/2025, he stated a reasonable amount of time to make the repair would have been two weeks from the time the supplies were purchased. He also stated the room did not present a homelike appearance.
