Summary

The facility failed to provide behavioral services from outside vendors in a timely and accurate manner for two residents. According to facility policy, the facility is responsible for obtaining and ensuring the timeliness and professional standards of outside services. For one resident, clinical records showed a psychological services progress note was documented on a date when the resident was not present in the facility, as confirmed by the Nursing Home Administrator. For another resident, a psychological services progress note was also documented after the resident had already passed away, as confirmed by the Director of Nursing. These findings were based on a review of clinical records, facility documents, and staff interviews. The records indicated discrepancies in the provision and documentation of behavioral services, with services being recorded for residents who were either not present or deceased at the time.
