Summary

The facility did not ensure that food was stored, prepared, distributed, and served in accordance with professional standards for food service safety. During a tour of the Food Service Department, surveyors observed 6 wooden pallets leaning against the wall in the outdoor receiving area and a large pile of black milk crates thrown to the left of the door. In the kitchen, the convection oven had a heavy build-up of black baked-on food particles on the inside surface, and the reach-in refrigerator did not have an internal thermometer to check temperature. On a follow-up visit in the dish area, the dishwasher wash temperature was 150 degrees and the rinse temperature was 120 degrees. The Food Service Director stated the booster heater was being repaired and that the machine had been used as a low-temperature machine with chlorine sanitizer, but the sanitizer was disconnected when the booster heater was installed. Under the dish machine, clear plastic tubing was found on the floor next to a gallon jug of household bleach. The Food Service Director placed the tubing into the jug and ran the machine, but the chlorine reading remained zero because bleach was not being pumped into the machine. The Food Service Director confirmed that the chlorine was not hooked up, that household bleach should not have been used, and that the chlorine level had not been checked before washing lunch dishes. The equipment and chemical company technician later confirmed the sanitizer pump was not operating when he arrived, that he got it working, and that the facility should use sanitizer for commercial dishwashing machines rather than household bleach. The Maintenance Director stated he was aware the dishwasher was being fixed and that it had been operating as a chemical sanitizer.
