Summary

The facility failed to ensure a resident admitted with a pressure ulcer received necessary treatment and services in a timely manner to promote healing and prevent infection. Resident R148 was admitted with diagnoses including chronic pain syndrome, severe protein-calorie malnutrition, systemic lupus erythematosus, rheumatoid arthritis, and infection and inflammatory reaction due to an internal left knee prosthesis. On admission, the resident’s initial assessment identified a wound on the left heel and left calf, bruising on the arm, and a reddened area on the buttocks, but it did not assess the pressure area on the lower back. The next day, a skin assessment identified a pressure ulcer on the lower spine measuring 4.2 x 4.2 x 0.1 cm and described as black and necrotic. Review of the treatment administration record showed that a physician order was not obtained and the wound was not treated until two days after admission. The physician order later directed cleansing the lower back wound, applying Santyl, and covering it with Border Gauze every evening shift. The resident and spouse stated the resident had been at the facility for days without any treatments, and the spouse said staff later documented wound treatment even though it had not been done. A nurse observed a treatment dressing on the lower spine that was dated earlier than the observation, and the DON confirmed the facility failed to initiate wound care the day of admission.
