Summary

The facility failed to maintain a complete, accurate, and readily accessible clinical record for one resident with diagnoses of weakness and difficulty walking. The resident had a current physician order dated June 9, 2025 for a restorative ambulation program to ambulate 50 to 100 feet with a rollator, contact guard assistance, and a wheelchair to follow, as well as a current order dated April 28, 2025 for active range of motion to both upper extremities for three sets of 10 repetitions. The task list in the electronic health record did not match the current physician orders. It showed a restorative ambulation task dated March 29, 2025 for ambulating 20 to 40 feet with a rollator, which differed from the current ordered parameters. The restorative documentation for the prior 30 days also included entries of not applicable on multiple dates, resident refusal on some dates, passive range of motion documented on one occasion, and ambulation and locomotion documented as completed on two occasions. The Director of Rehab stated the resident was currently receiving both PT and OT and should have been discharged from the restorative program before being placed on therapy, and that nursing staff oversee restorative therapies.
