Summary
The facility failed to provide a discharge plan for one of three residents reviewed for discharge planning. Review of the clinical record for this resident showed no evidence of a discharge plan, and a notification in the record indicated the discharge plan was overdue by 16 days. The resident's care plan did not address discharge preferences, and care conference notes also lacked documentation of a discharge plan.
