Summary
The facility failed to investigate an allegation of abuse involving a resident with severe cognitive impairment, anxiety, and depression. Multiple staff members, including a CNA and a CMA, witnessed and documented an incident in which a CNA shouted at the resident, with one staff member reporting that the CNA antagonized the resident who had expressed suicidal thoughts.
