Summary

The facility failed to provide necessary assistance with Activities of Daily Living (ADLs) for three residents who required varying levels of support. One resident with severe cognitive impairment and multiple medical conditions, including diabetes and cerebrovascular attack, was found lying in bed with feet encrusted in a yellow substance and a dirty mattress, indicating a lack of proper hygiene care.
