Summary

The facility failed to ensure appropriate fall interventions were in place for a resident with a history of repeated falls and impaired cognition, and failed to update the care plan after a fall. Resident #34 was admitted with diagnoses including repeated falls, diabetes mellitus, and protein-calorie malnutrition. The quarterly MDS dated 09/12/25 documented impaired cognition, and the care plan identified the resident as at risk for falls and potential injury.
