Failure to Provide Activities Based on Resident Preferences

New Lebanon Rehabilitation And Healthcare CenterNew Lebanon, Ohio Survey Completed on 09-30-2025

Summary

The facility failed to provide activities designed to meet the needs of one resident with major depression, COPD, vascular dementia, and right-sided spastic hemiplegia. The resident’s annual MDS dated 07/11/25 indicated severely impaired cognition and total dependence on staff for all ADLs. The MDS also reflected preferences for books, newspapers, magazines, music, keeping up with the news, group activities, and favorite activities, but the care plan for altered activity participation only noted that the resident preferred independent activities and did not include specific activities. The August 2025 activity log documented only one refusal of one-on-one and group activities and one group activity participation, with no other documentation showing the resident was offered or participated in activities during that month. The September 2025 activity log documented activity participation on four dates, with no other documentation showing activities were offered or participated in for the remainder of the month. Observations on 09/22/25 and 09/23/25 showed the resident remained in bed with no activities noted, no TV or music on, and no magazines, books, or newspapers present. The Activity Director stated the resident preferred independent activities and sometimes sat at the nurse’s station or joined group activities, confirmed the resident’s preferences were taken from past MDS assessments, confirmed family had not been contacted for preferences, and confirmed there was no documentation that activities were offered daily in August or September 2025.
