The quarterly MDS showed a BIMS score of 10 out of 15, indicating moderately impaired cognition. However, physician orders for August 2025 showed no active fluid restriction order, and the RDCS confirmed during interview that the restriction was no longer ordered but remained in the care plan and needed to be removed.
