Inaccurate PASRR Coding on MDS Assessment

Summary

The facility failed to accurately complete resident assessments for one resident reviewed for resident assessments. The resident had an admission date of 09/02/11 and diagnoses including schizoaffective disorder bipolar type, bipolar disorder, anxiety disorder, and dementia. Review of the significant change in condition PASRR identification screen completed on 01/18/22 due to a decline showed indications of serious mental illness and noted that within the previous two years the resident had used psychiatric services, including emergency mental health services and inpatient psychiatric hospitalization due to the mental disorder. The notice of PASRR level II outcome dated 01/24/22 showed the resident was approved for continued nursing facility services with recommended mental health services and supports put into place by the facility. However, the annual MDS assessments dated 01/27/24 and 12/13/24 both coded section A1500 for PASRR as no for the resident being considered by the state level II PASRR process to have serious mental illness and/or intellectual disability or a related condition. During interview on 08/27/25, MDS nurses and LPNs #660, #738, and #800 verified the annual MDS assessments were inaccurately coded.
