Summary

The facility failed to provide trauma-informed care for two residents with PTSD by not adequately identifying trauma history, triggers, or related interventions in the record and care plan. Resident #8 was admitted with PTSD, major depression, diabetes, and hemiplegia/hemiparesis following cerebral infarction. The social service initial evaluation documented that trauma history and triggers were unable to be determined or left blank, and the comprehensive care plan did not include PTSD, triggers, or trauma-informed interventions. Psychiatric assessments for Resident #8 documented depression, withdrawal, agitation, trauma/stressor related disorder, non-verbal behavior with staff and family without reason, and trauma from war. The psychiatric nurse practitioner noted a positive trauma history and probable PTSD based on the PC-PTSD screen. During interview, Resident #8 stated he had been abused at a previous facility and had served in the military for many years, including in a war. The Director of Social Service verified the social service assessments did not identify PTSD or triggers and that there was no care plan addressing PTSD, triggers, or interventions. Resident #74 was admitted with diagnoses including generalized osteoarthritis, generalized weakness, history of substance abuse, schizophrenia, and PTSD. The social service initial evaluation stated there was no history of trauma, left the trauma and trigger areas blank, and indicated no psychiatric follow-up had been scheduled or completed. The care plan completed after admission did not include PTSD care planning, and the resident remained without a PTSD care plan until it was later revised after the deficiency was identified. The facility policy required screening for PTSD history and symptoms during admission and routine assessments, with individualized care plans including known triggers and calming interventions.
