On 06/05/25, Resident #286 was pushed by Resident #201 in the dining room of the locked unit after Resident #286 touched Resident #201’s baby dolls. Resident #286 hit her head on the floor, was sent to the ER, and later returned with a large hematoma to the left side of the head and was treated for a closed head injury. On 08/04/25, Resident #106 and Resident #275 were involved in a physical altercation after Resident #106 wandered into Resident #275’s room; Resident #275 said he hit Resident #106 because of that. Resident #106 had a bump on the head and bruising to the upper and lower lip, while Resident #275 sustained a finger injury and an x-ray showed an acute fracture of the fifth digit. The facility concluded it could not substantiate wrongdoing by staff and stated the residents’ BIMS scores suggested they lacked the mental capacity to intentionally harm another, despite the resident statements describing why the assaults occurred.

She also stated she had concerns that the facility was not completing the ordered dressing changes and that the resident had worsening buttocks skin breakdown, while the DON stated treatments should be completed as ordered and documented on the TAR or in progress notes.
