QAPI Committee Lacked Infection Preventionist Participation
Summary
The facility did not maintain a Quality Assessment and Assurance committee that included the Infection Preventionist as a member, and the Infection Preventionist did not participate in or report on the Infection Prevention and Control Program or infection-related incidents to the QAPI committee meetings held from 03/31/2025 through 09/11/2025. The facility policy titled Quality Management/Performance Improvement dated 09/2025 stated that all staff participates in the performance improvement process and that quality improvement data, analysis, and discussion take place quarterly at team meetings. Attendance sheets for the QAPI meetings showed that the Infection Preventionist did not attend the meetings held on 03/31/2025, 06/18/2025, and 09/11/1015. The committee member list provided by the facility did not include the Infection Preventionist. During interviews, the Medical Director stated they began facilitating QAPI meetings in September 2025 and were not aware the Infection Preventionist was required to attend. Infection Control Practitioner #1 and #2 stated they had never attended QAPI meetings because they were still in training, and the Assistant Director of Infection Prevention and Control stated they missed the meetings due to vacation or leave. The Director of Infection Prevention and Control stated they were a member of the committee but did not attend the September 2025 meeting because of another administrative leadership meeting and could not recall why they missed the June 2025 meeting. The Administrator stated infection control issues were discussed in daily morning meetings and acknowledged it was an oversight that the Infection Preventionist was not listed as a committee member and did not attend the quarterly QAPI meetings.
