Resident #163 was re-admitted with diagnoses including atrial fibrillation, heart failure, and hypertension. The resident’s care plan addressed potential for bleeding related to anticoagulation therapy with apixaban (Eliquis), and the record showed apixaban was administered in early July, then held and discontinued, while aspirin was later ordered and administered instead.
