The facility failed to routinely post daily staffing sheets at the public entrance and failed to accurately post the number of hours worked by staff who provided care. The facility census was 89. During an email correspondence, the administrator stated the facility did not have a policy specific to posted staffing, but expected the facility to follow the regulatory requirements for posted staffing. On observation, the plastic sleeve at the entrance where staffing was displayed for public view was empty. Review of multiple daily staffing sheets and the corresponding employee electronic time sheets showed repeated mismatches between the posted hours and the actual hours worked. On 1/1/26, the posted sheet listed CNA hours that did not match the hour details report for both day and night shifts. On 1/2/26, the posted CNA hours again did not match the actual hours worked. On 1/3/26, the posted sheet listed RN and CNA hours that did not match the time sheets. On 1/4/26, the posted CNA hours did not match the actual hours worked. On 1/5/26, the posted sheet listed charge nurse, CMT, and CNA hours that did not match the time sheets. On 1/6/26, the posted sheet listed charge nurse and CNA hours that did not match the actual hours worked. During interviews, the DON said she was responsible for posting and updating the staffing sheets and acknowledged they were not always posted for public view and were not always updated to reflect actual hours worked. The Administrator later stated the hours listed as worked on 1/6/26 were incorrect and that two additional CMTs were called in to cover the shift, then wrote those names on the staffing sheet during the interview. However, both CMTs interviewed said they did not work at the facility on that date, and the hour details report showed they did not work that day. The Administrator then stated she had mixed up the days and that the two CMTs were added to the staffing sheet in error.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

The facility policy, Nurse Staffing Posted Information dated 02/01/2026, stated that nurse staffing information was to be made readily available in a readable format to residents, staff, and visitors at any given time.

Surveyors observed the posting on multiple occasions and found it dated 5/26/26 on 5/27/26 at 2:15 p.m. and again on 5/28/26 at 6:58 a.m., dated 5/26/25 on 5/27/26 at 4:10 p.m., and dated 5/28/26 on 5/29/2026 at 8:13 a.m. The deficiency affected all 24 residents residing in the facility. During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
