Summary

The facility failed to store soiled linens and transport laundry in a manner that prevented the spread of infection. During an observation on 1/14/26 at 8:16 a.m., a laundry assistant carried an arm load of clean laundry on hangers, draped over the arm and against scrubs, into an unidentified resident's room. Later that morning, the laundry room was observed with two filled blue bags on the floor in front of containers, four plastic bags filled with mop heads and rags on the floor by the door to the washing and drying room, multiple used cleaning rags and three dirty mop heads unbagged on the floor, and two plastic bags filled with residents' clothes on the floor by the back wall. The laundry assistant stated she was the only employee for the laundry department and confirmed the items on the floor, stating she relied on staff to place items in proper bags and containers. She also stated the clean laundry had been transported to the resident room incorrectly and should have been covered on a cart during transport. The infection preventionist confirmed the observations and stated the soiled items on the floor were against policy and procedure for handling laundry. The DON stated the expectation was for staff to follow the policy and procedures for laundry, and the facility policy directed staff to transport and process laundry according to best practices for infection prevention and control, with soiled laundry bagged and placed in proper containers.
